Accessing Care at Lionel Kelland Hospice

For an individual and their family, facing a terminal illness is never easy. But you do not have to navigate this journey alone. Lionel Kelland Hospice provides residential hospice end-of-life care at no cost. All members of the community are welcome regardless of race, ethnicity, political affiliation, religion, age, sexual orientation, gender identity or expression, family status, marital status (including single status), economic circumstance, or disability.

If you are considering entering our hospice as a resident or are a family member or health care professional acting on behalf of a potential resident, we are here to provide information, answer your questions, and help start the admissions process.

To become a resident at Lionel Kelland Hospice, the first step is to speak with your healthcare professional (family doctor, nurse practitioner, palliative care consultant, etc.) about whether hospice care is the right choice for you. If a mutual decision to enter the Hospice is reached, our referral form will then be completed by your healthcare provider on your behalf.

Lionel Kelland Hospice will then review your application. If accepted, we will contact you once a room becomes available.

  •  Resident or their Substitute Decision Maker must provide consent for the admission and agree with the goals of care offered at Lionel Kelland Hospice.
  • Must be an adult living with a life-threatening progressive or terminal illness and be assessed as having a life expectancy of less than three months.
  • Palliative Performance Scale (PPS) of 30% or less.
  • The person requires care that can be safely provided outside of an acute care facility.
  • Must have a valid Health Card.
  • Must have a Do Not Resuscitate Certificate (DNR).
  • Resident Care must be assumed by the Medical Advisor or Hospice On-Call Physician.
  • Confirm that the option of a home death has been considered and is not deemed feasible or desirable by the applicant.
  • Palliative home supports options have been explored and not determined appropriate.

General exclusion criteria includes, but is not limited to:

  • Patients requiring interventions or management strategies that are not provided in hospice (i.e., artificial ventilation, etc.)
  • Patients with wandering or aggressive behaviors that threaten their safety and/or the safety of other residents and staff.
  •  Patients with active, infectious diarrhea (i.e., clostridium difficile) or active airborne disease (i.e., measles, tuberculosis, etc.)
  • Invalid Health Card.
  • Patients who are actively dying and are admitted in another care facility (i.e. LTC and death is anticipated within 24 hours).
  • Patients and/or families who disagree with the Lionel Kelland Hospice Admission Agreement.
